How to make a claim

For each claim, a fully completed claim form must be submitted to us. The claim form must be completed and signed by your physician and sent with the original bills and receipts for the treatment received. You have 180 days to submit your claims.

  • In patient / day-patient treatments
    After doing your pre-certification, you will need to send the original claim form and the unpaid invoices ( if given to you by the medical facility) to our claims department (see claims dept. contact info below).

  • Emergency admission
    In the event of Emergency admissions, You should contact the Medical Helpline as soon as possible after admission and follow the steps described earlier for In-Patient Treatment. Failure to contact the Medical Helpline will result in Treatment or any Evacuation cost only being reimbursed at 50% of the costs incurred within the terms of the Policy. Please do not delay obtaining Emergency Treatment.

  • Outpatient treatment
    If You receive medical Treatment as an Out-Patient, Treatment must be paid for in full by You before claiming reimbursement. In such instances please ensure that a claim form is completed by You and the Physician or Specialist Physician. Please remit this to the Goodhealth Claims Department with all substantiating proof of Your claim, including the original invoices and receipts, prescription and a written diagnosis from the Physician.

Please use the claim form specific to the plan under which you are covered.
