Initiate a pre-certification

For any planned hospitalization (non-Emergency In-patient / Day-Patient treatment), MRI and CT scans, we should be notified as soon as reasonably possible prior to admission, in order to avoid misunderstandings about the insurance cover.

You have different alternatives to pre-certify:

1. You can call our medical hotline at (00) 1 305 443 6267 or use our Toll Free number 1 800 914 2176 if you are calling from inside the USA
2. you can complete the Pre-certification form online
3. or download the form as a PDF and then fax it to us at 305 443 6648

   You will be required to give full details of the condition, proposed treatment (including dates and name of procedure if known) together with the name of the specialist physician and details of the medical facility. We will confirm your cover and will dispatch written confirmation to you.

   We will attempt at all times to make arrangements with the Medical Facility for all eligible bills to be settled directly when using a Preferred Care Provider. Remember that payment of all expenses incurred by you will only be reimbursed at 50% of the costs incurred if you do not follow this procedure.
